OW Client Referral Form

Use this form to refer an Ontario Works client to the Halton Food Connect program to learn more about monthly food box deliveries. Once submitted, a member of our team will be in touch with the referred client within 1-2 business days to share more information about registering for the program, what to expect, and answer any questions they may have.
